Never submitted an MIS Request Form? This guide will walk you through where to find the form, and how to fill it out.
Environment
EC MIS Self Service Portal - Client Portal Home (ecpowerslife.com)
Resolution
- Go to the EC MIS Self Service Portal home page - Client Portal Home (ecpowerslife.com)
- Now, you can either click on the shortcut under the "Create an MIS Request" category for MIS Equipment Request / New Account, or navigate through the Service Catalog by clicking on the Services tab on the header. Reference the picture below:

- Once you are on the MIS Equipment Request page, be sure to fill out all fields applicable to your request. If you see a red "*" next to a field, it is mandatory to fill out information.

- If any of the fields do not represent important information about your request, include it in the 'Other Information' field. Your diligence in including all pertinent information to your request saves time and communication between your team and MIS helping us streamline the request process.

- Once all your information has been entered, click 'Submit'. You should receive an email confirmation for your request, a ticket will get created and your request will enter the queue for review. If you have any questions, respond to the email confirmation for your ticket that you received.
